Responsibilities
- Administer medications and provide routine nursing care per the patient care plan
- Monitor patient status, document vital signs, and report changes to the supervising RN
- Assist with activities of daily living and essential comfort measures
- Perform wound care, dressing changes, and basic procedures as directed
- Adhere to infection control practices and safety protocols
- Collaborate with the interdisciplinary team to implement and adjust care plans

Requirements
- Active LPN license in New Hampshire
- BLS/CPR certification
- Schools or employers may prefer 1 year of LPN experience in a skilled nursing or long-term care setting
- Ability to work an evening shift (5x8) and meet assignment requirements
- Ability to pass standard background checks and health clearances
- Strong communication and teamwork skills essential for coordinated care
