Carle Health is seeking a dedicated Pharmacist 2 to join our Healthcare & Medical Services team within our integrated care system. In this role, you will provide patient-centered clinical pharmacy services that support safe, effective, and evidence-based medication use across inpatient and outpatient settings. Working as a key member of an interdisciplinary care team, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy, improve patient outcomes, and uphold Carle Health’s commitment to quality care and patient safety. The Pharmacist 2 is responsible for verifying and dispensing medication orders, performing comprehensive drug interaction and allergy screenings, and ensuring accurate dosing based on clinical guidelines, lab values, organ function, and patient-specific factors. You will provide direct patient counseling on medication indications, administration, side effects, adherence strategies, and monitoring parameters, with a strong focus on clear communication and health literacy. Additional responsibilities include performing medication reconciliation at transitions of care, supporting formulary management, and serving as a drug information resource for providers and staff. You will prepare and oversee sterile and non-sterile compounding, including IV admixtures, following USP standards and internal policies to maintain the highest standards of safety and quality. The role requires proficient use of electronic medical records, pharmacy information systems, and automated dispensing technologies to ensure accurate documentation, efficient workflow, and compliance with regulatory requirements. You will actively participate in quality improvement initiatives, medication safety reviews, and adherence to Joint Commission, CMS, and state board of pharmacy regulations, reporting and helping to analyze medication incidents to drive continuous improvement. You may be involved in precepting pharmacy students or residents, supporting staff education, and participating in interprofessional rounds as needed.
